Waiver & Release of Liability

By signing this agreement, I acknowledge and agree to the following:

Voluntary Participation

I declare that I am over 18 years of age (or have otherwise provided parental consent) and acknowledge and understand that I have voluntarily chosen to participate in the classes and activities offered by Balance Beechworth. These may include physical activity such as Pilates, yoga, stretching and breathwork.

Assumption of Risk

I understand that participation in any physical activity involves inherent risks, including but not limited to muscle strains, sprains, falls, or other injuries. I acknowledge that I am solely responsible for assessing my physical and mental fitness for participation.

Medical Clearance

I affirm that I am in good physical and mental condition and do not suffer from any medical condition that would prevent my safe participation in these activities. I understand that it is my responsibility to consult with a GP prior to beginning any fitness classes if needed. I will inform my instructor prior to class if I have any injuries that may affect my ability to perform some exercises and take responsibility if I choose to partake in a class that may exacerbate said injury.

Injury and Emergency Response

In the event of an injury, illness, or emergency during participation, I authorise Balance Beechworth staff to obtain medical treatment on my behalf if deemed necessary. I understand that I am responsible for all costs associated with such treatment.

Respectful Conduct Policy (Zero Tolerance)

Balance Beechworth is committed to maintaining a safe, respectful, and inclusive environment for all clients and staff.

I agree to:

• Treat all instructors, staff, and fellow clients with courtesy and respect at all times.

• Refrain from any form of harassment, bullying, discrimination, or disruptive behaviour.

• Respect personal boundaries and the shared studio space.

Cancellation and Refund Policy

To avoid being charged the full class fee, cancellations must be made at least 12 hours before the scheduled start time. Cancellations must be completed through the Wellness Living app or via our website. Please note, we reserve the right to deny refunds.

Dishonour Fee

A fee of $8.80 will be charged by Paychoice (our payment provider) for any failed transactions due to insufficient funds. This applies to both direct debit payments and one-off class pass purchases.

Release of Liability

I hereby release, waive, discharge, and hold harmless Balance Beechworth, its owners, instructors and employees from any and all claims, liabilities, demands, actions, or causes of action, related to any loss, injury, or damage that may arise during or as a result of my participation.

Photography/Media Release

I do grant permission for photos or videos taken during classes to be used by Balance Beechworth for promotional purposes.

Acknowledgment of Understanding

I have read this waiver fully and understand its contents. I understand that by signing this form I am giving up certain legal rights.
